We are in-network with several insurance companies. Blue Cross Blue Shield, Select Health, PEHP, DMBA and University of Utah. We encourage you to contact your insurance company directly to determine if we are in-network for your specific plan.
We recommend asking your insurance company the following questions to help determine your possible benefit and coverage:
- Does my health insurance plan include mental health benefits? If so is there a separate network I am required to use?
- Do I have a deductible, or copay, if so, what is it?
- Does my plan limit how many sessions per year I have and if so what is the limit?
- Do I need written approval from my primary care physician or from the insurance in order for the sessions to be covered?
If we are not in-network with your insurance company, you can check with your insurance provider or employee benefit plan to see if services may be covered in full or part through an out-of-network benefit.
If you elect to utilize an out-of-network benefit please note that you will be responsible for the full cost of the session at the time of service. We will email you a "superbill" that you can submit to your insurance company for any possible reimbursement. We are not able to submit insurance claims for out-of-network insurance companies.
We accept cash, clergy pay and all major credit cards.
All clients are required to keep a credit card on file.
Payment is due at time of service for self-pay clients. For insurance co- pays are due at time of service. Any balance remaining after insurance has paid their portion will automatically be charged to the credit card on file and an invoice will be available in the client portal.
